Tribute Notification (if applicable)
Minimum donation of $18 required for notification of tribute. A lovely tribute card will be mailed to the person/recipient you identify below to make them aware a donation was made in their honor or in memory of their loved one.
 

(First and last names are required if tribute is for an individual)
Recipient Information (Person to Notify)
If the recipient is a current member of Emanu El, we will have their address on file.  
*If the recipient is a non-member, we require their current address in order to notify them of your donation. If you are unsure about their membership status, please include their mailing address. 
Not required - tribute cards/notifications are mailed via USPS
*Required for all non-members of Emanu El
NOTE: If multiple people need to be sent a tribute notification, please make a separate donation in order to include their individual mailing information. 

Example:  If you want Emanu El to notify two recipients with two separate addresses, please make two donations and list the recipient for each one. We should have one address per dedication.  If the total donation is for $36, make two donations for $18/ea.
